Seems like, in doing some reading, you may have a calcium deficiency. Have you tested your calcium level? Other possibilities are sudden temp swings, Ph swings. For your water changes are you mixing your water 24 hours prior to the change? Are you matching the temp of the change water to your display water? Are you aerating the water to balance Ph.
The other possibility is a bacterial infection which Euphyllia coral as susceptible to. Have you added anything new to the tank recently that may have carried something in?

if the heads are falling off the stalks, your expiriencing Polyp bailout, a last ditch survival response for a coral that finds itself in unfit conditions. Euphyllia are one of the many corals that can accomplish that. place the ditched heads in a low flow area, they will eventually grow their own skeleton, if you can keep them alive long enough.

unfortunatly that leaves many possible options for the problem.

Lighting, water quality, flow. (lighting is the least likley possibility)

I would suggest its a water qualiyt issue if your water is cloudy there is a problem somewhere. test all your parameters. PH, alk/kh, calcium, am, nitrite, nitrate, phosphate, etc. the more exact conditions you can list the better chance any one has of helping you find your problem.

also, I see a rather good size clown in the tank, is the clown hosting in the frogspawn? a large clown hosting in a frogspawn can seriously damage the coral and possibly cause bailout.

did you ever figure out what the problem was? I'm having the same issue! My frogspawn I have had for over a year and its ALWAYS been the healthiest coral i've ever had! its gone through multiple disasters and always pulled through! its HAD 4 hears on it.. two have already fallen off... trying to figure out the issue before the other two die! help!!
