can you get a better picture.
Move him to a hospital tank, get the temp as low as you can close to 68 degees.
Where is the horse housed, what else is in the tank? What are your water readings.
you will need to treat with antibiotics - do you have any on hand

Ditto on Theresa. It's really hard to tell with the pic you provided, but if it's a true hole in his stomach then you're probably looking at a heavy bacterial infection. Tank by himself to prevent contamination, temp very low to slow the bacteria breeding, and Furan-II if it's a vibrio infection.
