You can do 1 qt and medicate if needed. Ich in most cases is the most common issue. Its also the easiest treated in a qt. Either with hypo or copper

I'd say a month. Ich isn't the only potential issue and you want to have them in QT long enough to be sure nothing is going to crop up.

When the fish is in QT, you really need to give them a close look (preferably with a magnifying glass) at least a couple of times a day, at different times of day. Otherwise, something like ich can go undetected. For parasites, best to leave the fish in QT for min 3 week. Its not a bad plan to perform hyposalinity on new fish regardless of ich presence.

So when In doubt do hypo salinity and then copper if it's bad

Ich is the number 1 parasite seen in the hobby so it is more likely than not that any fish you acquire is at high risk. Hypo is harmless and even helpful for a new fish due alleviation of metabolism workload caused to the fish in order process salt water. Added bonus is that it causes increased appetite which is a good thing for a stressed fish that may have disease. You do need to administer it appropriately, otherwise it won't work and could be harmful. Guide here:
