    It can in a reactor if the particles spend too much sitting still. Unlikely with the amount of GFO you’d put in a HOB filter. Use less and Change when needed.

    In my opinion all fish should be quarantined. The fish are not quarantined prior to sale and the wholesalers tanks are infested with diseases. You got lucky once. You won’t always be lucky.

    Your salinity(specific gravity) should be 1.025-1.027. Less is fine for fish short term but not long term. Inverts like anemones need the 1.026 salinity to survive. I doubt your nem is thriving, if Conditions, water quality, salinity and lighting aren’t perfect the nem will not survive.
