    Well it sounds like your problem just got figured out. For me it was just a guess. Sounds like snakeblitz has more knowledge in the area. I'd take his advise. If it were me I'd probably do a hefty water change then like he said stop adding the extra carbon to the system.

    Yeah and like bang guy said that 2 inches could be adequate if you have something to move the air. Is there a canopy/ lid over the tank. if so a very small fan positioned over the gap could work. although without a large hood to hide it that may be rather unsightly.

    I'd just leave it fish-less for a while. Don't add medications or chemicals to the tank. You have plenty of good stuff living in there that you don't want affected by doing that.

    Yeah hopefully it's not sick/diseased. Typical quarantine period for marine fish is 30 days.. and if after 30 days you see no sign of sickness then you should be good to put in your main tank. If you're right around the 30 day mark and are seeing this then you may have a sick fish on your hands...

    Check your temperature as well... higher temps can can result in higher metabolism increasing the need for oxygen and at the same time reduce the dissolved oxygen levels in the tank resulting in higher respiration.

    You want to acclimate him properly. Drip acclimation for him to slowly adjust and get used to all of your water parameters for a period of one to two hours as well as temperature acclimation.
