Abrassion on back of purple tang

I'm currently performing Hypo on my fish, and so far, everything has been fine, but then this morning, when the lights came on, the purple tang has worn away his coloration on his dorsal (only towards the back) and then has an abrased area on his back in the same area... Any suggestions on what this could be? Could he rub his back up into a rock hard enough to do this? Or is it something else? Thanks.
I also just noticed that he has missing piece of fin on both side (pectoral) fins... It looks like a missing piece anyways. I've never seen him fight with anything else, and it seems odd that everythings identical on both sides. Thanks again.

Well, levels are 0,0,0, in the main tank, but took out all rocks and inverts to 60 gallon tupperware with vho.
Actually, what's funny, is now, when I just got home from work, it is almost all gone. Just one little area of whitish-clear near the top of the dorsal. Very strange. Haven't been able to get a good look at the pectorals yet.
If it does end up being some sort of bacterial infection, such as finrot, are there any medications that are safe to use in the main tank that can be removed prior to placing any inverts/live rock back in tank? I'm referring to non-ick type stuff; Are the anti-biotics safe? Like tetracycline? Not very familiar with all the different meds...
Of course, I'm not sure if there is a need to medicate yet. I'll try to get a picture and post it.
Amd yes, I am buffering pH. It is currently at 8.02
