If you can get behind it you should be able to. I would brush/roll it though instead of spray paint.

I have been thinking about doing this so I have been reading up on it. Most of what I have read makes me believe that it is safe. The main thing that I have read says to not use any type of spray paint, and be very cautious when painting near the top. Making sure your tank is sealed with painters plastic and tape is probably a good idea as well.

As long as you can reach back there, it shouldn't be a problem. Just tape off any areas you don't want to get paint on. Like a sump for instance.
Depending on how large your tank is I would use a 4" foam roller. Latex paint ought to work. I wouldn't use a regular size roller as it will probably be too hard to control behind the tank. Also you can get a pole to attach to a roller making it easier than using a brush.

You can also if able to reach back there use a background and sea view gel. It stays wet so whenever you wanna change backgrounds can peel them off and put on a new one. You put it on and then squeegee it to smooth it out with a credit card or other flat plastic or a ruler, so on. Just a thought.
