I would guess he is hours from death. It looks like he has a gray hayze forming in the top of his eye and he also has a small orange patch on his back in the middle

Looks pretty thin too, or is it just the pic.? Did you search the disease threads? Sorry I can't be of more help. Some questions you will undoubtedly get are: How big of a tank, post parametrs of water and how long tank's been up, how long you had the fish. HTH

The fish is starving for sure!! It looks like some fungus or bacterial infection, if he was not so emaciated there might of been a chance.
If you replace him you need to feed them and give them algae to eat always available. Tangs consume a lot of food.

if not dead already real soon, i dont mean to be harsh but you need to get him into a QT asap!!! should already be in there, some kind of fungal bacteria as well as starvation....get him a lettuce clip asap and some greens in there, he needs to graze....and limit your lighting, with him only a couple hours a day, sleep when youre sick right? same thing he needs to recover, maybe some stress coat, i dunno this is not looking good
LETTUCE CLIP!!! he needs to graze on greens all day
