I've had my regal tang in my tank for 3 weeks now and I just noticed today that one of his eyes is puffy and white. Did a water test and all tested great. His tankmates are 3 spotted cardinals, 1 firefish gobi, 15 hermit crabs, 12 snails, 1 urchin and thats all. Could they be harassing him as well. Don't know if this has anything to do with it but the top off one of my feather duster fell off yesterday, but the other one seems fine. What should I do next. I have a FOWLR

Do you have a separate QT? He could be treated with antibiotics, but not in a tank with LR or LS.
Otherwise, try doing a water change and feeding the fish garlic soaked food. Keep an eye on the tank to see if you note any harrassment.

2 days later his swollen eye has gone away but now he has a white spot on his pectoral fin. What could this be?

That is what I was afraid of. Cloudy, puffy eyes, particularly with a new fish, usally means ich.
All of your fish have now been exposed to the parasite, which is hightly contagious. You will have to set up a hospital tank to treat them, since you have inverts. Do you have live rock as well?

Just don't want to believe this. He seems so healthy. Eating, picking at algae, swimming normally. Will the garlic soaked food help. How 'bout a cleaner shrimp?

Yes, go ahead and try the cleaner shrimp and the garlic soaked food. NOT a cleaner wrasse however. Hopefully this will address the situation since the problem seems very mild.

I have a seaclone 100 for my 55 gallon tank. I did the modifications for it and now works pretty well. I set up the hospital tank just in case and did the water change. His eye is fine now and just has some crap on one of his fins. Should I move him to the hospital now or wait for more white spots. By the way, does anybody have a photo of what ich looks like on a fish. I've searched but can't find anything

A couple post up by Mikester311, white spots on fins (subject) shows what looks to be ich. Also if your fish are "scratching" on the LR or substrate thats a sure sign. HTH
