Condi Anemone Died. Why?

My Condi died yesterday, It was hanging on my rock, where it had been for about 2-3 months. Then I got home and it was laying in the front of the tank all shrivled up and the base of the anemone looked like it had been tore. It had a hole in it, with stuff hanging out. It was doing great until last night. When I got it, it was bleached white. About a week went by and it was to a good healthy brownish color. What could have happened? My only guess is my LTA and my condi had some kind of a chemical fight. I'll post a pic of their postitions in the tank. I have a 30gl. 150 watts MH 130 watts PC actinic. Water conditions are

My Filtration is HOB mechanical filter and a HOB protien skimmer (for up to 75gls). 65-70lbs LR. 1.5 inch LSB. Any help would be great.

The only thing I can think of is that it got melted/burned by your heater? Thats what happened to my condi.

I don't use a heater. My tank stays at 78-82 degrees, so I don't use one because I don't trust them. (<Long Story.) Thanks for the help though, greatly appreciated.

Can you maybe think of anything else.
