Death of a Lionfish

Just found out I lost my Fuzzy Dwarf Lionfish today.
I came home from work and he was laying on the bottom upside down. I removed him and then checked my water. PH-8.3 Salinity-1.023 Nitrates and Nitrites-0 Amonia-0. He was in a 150G with a blue tang, flame angel, foxface, and a snowflake eel. He seemed to be acting funny the last few days. He refused to eat whatever I tried to feed him. This wasn't the first time he had done this so I wasn't overly worried. His favorite food in the past was silversides. I had to break him of only eating those. Any ideas as to what could have happened?
He was one of my favorites and my 6 year old just loved him.

How old was he?
It sometimes just comes down to the natural cycles of life...
If there isn't anything wrong with your parameters, he could have been depressed?
What were you feeding him before he died? Maybe he wasn't getting enough nutrience?
