dying flame

I just lost my 5th angel over the last 2 years. All other fish are fine and been in there at least 6 mnths I cant keep a flame mopre than 2 weeks . Does any one else have a problem with them?

Is this fish eating at the LFS when you purchase him? How do you acclimate him? What are your water parameters (ammonia, nitrite, nitrate, kH, pH, salinity, and temp)?
