Fish keep dying

So iv set up my tank got it all running but my fish just keep dying i have a blue tang and a small cleaning fish in there im not sure of his name, iv had them from the start and they are fine but iv had a lion fish who was fine for weeks just up and died for no reason then i had an bi-colour angel who was also fine for a good 2-3 weeks then just up and died aswell i have no idea if its just a coincidence or what all my water levels are fine my salt is perfect water temp all my corals are perfect so i am really stuck can anyone please help.

I think an overall description of your setup might help to start. What do you test for and what are the test results? Salinity? Etc...

Dido need all the facts on your tank and how you aclimate your fish, if you QT them. What was happen B4 time of death, were they eating, were they swimming normal, was there any markings on there bodies, were there eyes clear or cloudy.
