Fish keep dying

I just don't understand why my fish keep dying. I am watching my Sailfin Tang die as I type this message. When I introduced him into the tank, he seemed fine. Swimming around and eating. Not really hiding much at all. Yesterday he hid a little bit. Today when I looked at him in the morning he is lying/proping himself against a rock and breathing hard. His color seems faded. He has only been in the tank since Sat. This is exactly how my yellow tang died. All water parameters seem good
Temp 78-80 depending on time of day
SG 1.025
Amm/Nitrite/Nitrate 0

Thanks for the quick post terryb. My PH is 8.2. I red up on velvet and ick. I don't know that I see either of those in this case. He is faded which could be mistaken for velvet; however I have seen no scratching. There are a few other fish in this tank and they seem normal. However I have lost several fish in this tank. The clown and damsel are fine. I also have a Kole tang in the tank. The Kole has a spot on his left side where one of his fins beats against his side. Not sure what this is but for now he is acting as he usually does. He seems to feed well...he doesnt really eat that much flake food or seaweed...but he constantly picks at the rocks. He also seems to begin feeding on the algae on the rocks when I feed the other fish...kinda interesting I guess.

Ok...update....
I was able to more closely inspect my fish and I believe he has ick. My fish has 10-12 spots per side. From speaking to my LFS, I don't believe that the number would represent a significant enough number to cause the fish stress...but rather that the fish is stress and is showing signs of ick as a result. I FW dipped him for 4 minutes but he doesn't appear much improved.....a few hours later....RIP
