Flame won't eat

My flame angel won't eat much. It is in my QT and I had to perform hypo due to it iching itself. I completed the 3 weeks at 1.009 last week. I am currently raising the sg and I am at 1.017. I have had numerous problems with high nitrite levels but have them at 0 now.
Back to the Angel. It has lost a lot of weight. It is very lethagic and doesn't swim very fast at all. I have tried every food I have trying to get it to eat( frozen formula 1, formula 2, prime reef, angel formula, mysis shrimp, brine shrimp, various flake foods) I have added selecon and garlic to most of the food that I have feed him. I have no access to live food. Any advice.

I am using a ATP refractometer.
It started about a week into hypo so it has been going on for 2-3 weeks.
Like I said I did have a nitrite problem but fixed it by doing water changes.
At first the angel would take some of the food into his mouth but would spit it out. The last week he/she can sense the food and will actively seek it out but once he/she finds it does not seem interested.
Like I said the fish is sickly skinny

First, I realy wish that you had brought this to our attention sooner but at least we can try to help now. Are the gills red at all? Do you notice the fish mouthing for no reason, the mouth is opening or gaping without the gills moving?

No he is not gasping at all, but now that I have looked close there are some small black spots(about 6 of them) protruding out by his tail fin. They look out of place because they are a dull blackish brown.
By the way he is pooping so I guess he has ate something.

Check out the Diseased fish FAQ for pics of black ich. Hypo would not have cured that and it would have caused scratching. Formalin is the treatment for that.
