HELP! clam seems to have lost its foot

Ive had my blue maxima clam for maybe a little over a month now, all was fine it looked great! until yesterday. I came home and it had completly turned itself and would open up about halfway for 5 or ten minutes then kinda lift its self up(while being closed) and turned some more.
I was no able to get on the ocmputer last night. I prepared a water change last night and did it this morning before I went to school. Came home, turned lights on and barely opens up, all my SPS corals look fine.
I lifted up the clam to try and put it back in the position it was doing well in and there was a white small muscle/tendon looking thing that appeared to be its foot. During this process I do no think I
was the cause of the foot coming off. Could this have been from something in the sand? I can see directly through his mouth and see the sand.
Nitrates were barely
noticable last night, that is no usual.
Today, after the water change, paremeters are:
nitrates-0
nitrites-o
my alkaliity was a little low, I had just added bionic.
ph-8.0-also a little low, Im thinking because of the water change.

the same thing happen to me ....i just got a clam about 2 days ago and placed it up on a flat rock and it seem to be fine for a day and the next day it had moved???? a little bit over and i saw the same thing (white tissue looking thing) but it seems to be okay but im not sure? I hope someone else will respond to this thread with more info!!!!

maybe I typed too much...
mine hasnt really opened up at all since I moved it today. Its probably 98% closed.

a pic would help. if u can see through the mouth and onto the sand, it means the clam is gaping. when it gaps, its usually stressed or dying.

well now the front half of the the clam has unravealed from the core.
When I cam home and saw it was trying to move itself, it has one one time totally turned its self around. Prior to this he had stayed in the same spot and looked great, nice and open. I feed the tank probably twice a week.
I dont have a camera on me right now, could something in the tank like a shrimp or something else lurking in the tank could has caused stress?

I really dont think it would be. I have a 29 with two koralia 1s so thats what 800gph? and both of those are pointed more on the rocks.
Plus would it really take a month for it to start getting stressed?
I do have a rusty angel that will nip at a coral here and there...
my gf has a camera, im not too big on things like that, but now I wish I sure had one....
