Help my Sick fish please!!!!!!!

Ok he looks eather sick or beat up eather way he is out of the DT into a QT he will eat but I want to take no chances. I also saw him scraping agent rocks like he was trying to scratch an Ich. It looks like his scales are comming off? The nick on his right fin was there befor. Nothing is rotting off.....yet. Here are some pics any help would be great.

I am treating him with Pimafix for Antirungal and Melafix for Antibacterial
