Help with a Coral

I bought a coral about a week ago and it was doing fine until 2 days ago. I think it is called Turbenia. It had small polyp growing from the plates but they went away about 2 days ago. There is a small plate that is growing behind it and those polyps are doing fine. I tested my water and all levels are normal. I did a 20 gal. water change yesterday. It also has a web like substance on part of it. How can I make it better? What could be the problem? Please help.

How long has your tank been cycled? Please post your tank levels: Ammonia, nitrite, nitrate, ph, alkalinity, calcium, temperature, specific gravity/salinity.
What was your acclimation procedure?
