Humu Humu top trigger getting ragged

My humu humu trigger fish's top "trigger" is getting ragged with just a hint of grey or white (probably a bit of dead tissue.) Tail and other fins look fine.
Anyone have this happen? Is it a start of a fin rot and should I get her out and into hospital tank now?

Our Humu Humu has taken a conch shell as "home" and likes to wedge in there so that is probably what is doing it. We will keep our eyes on it.
