ick,no ich,ich??!!Please help(puffer)

I have had this porcupine puffer for about two months now,since he was very small about 1.5".Anyways after a few days he developed four white dots on his tail and I proceeded to feed him garlic soaked food(I, ignorantly, do not have a hosp. tank)
the spots seemed to blend right into him and disappear,this process has happened about four times;He eats like a pig and has AT LEAST doubled in size.The Valentini who has been w/ him the same amount of time has no spots,never has.My lfs says to ignore this,but come on, I cant..anyone have any ideas?Thank you, I'm desperate!

Thank you for your true advice/help,unfortunatley when I awoke this morning my Valentini,who showed zero signs,was dead as a doornail.My porky is now covered in spots and not eating and my lfs is now closed..I'm expexting the worst and hoping for the best.If in fact my porky does die it is min, 30 day waiting period for any new fish,right?Thank you again.
