i'm not sure if this is ich, please help

i have a blue tang and he has white spots but they are more like bumps that look like they are under his skin rather than on the outside like ich usually is. what is this inside it? it comes and goes too, somedays he is really bright blue, other days he has these whitish bumps. i feed it flake and seaweed selects and i use garlic 3 times a week in the food.
also, i do not have a hospital tank setup, nor will i be able to get one soon, so what should i do if it is ich?

i saw a couple posts of other people with the same problem so i will clarify that these look like zits under the skin, not like grains of salt. the other fish i have are fine, not a spot or bump on them, but i am fearing that will change, but i'm hoping it won't

How many spots do you see?
Also, what type of tank do you have? FO or do you have live rock and inverts as well?

about like the first pic, 20 maybe. they dont look like that tho, these are under the skin, you can actually see the skin of the fish is bumpy. i have live rock and inverts in my tank, thats it, no reef stuff.
