Once that Thalassoma wrasse reaches maturity, it's most likely going to become a real bully, esp. since it's "undertanked". Your P. radiata lionfish is a prime victim, since they're extremely mellow...I've seen banana wrasses harass lionfishes to death in the past.
Also realize that torpedo-shaped fishes such as your goby are easily swallowed, so your lionfish might decide it looks tasty. Your clownfish is at risk as well. In the wild, P. radiata tends to feed mostly on crustaceans, but they won't turn down a fish if they're hungry.
Butterflies and angels are browsing fishes, so they're just doing what comes naturally when they pick at the rocks. In fact, if you're not offer the angelfish some form of vegetable matter (nori/algae sheets, blanched leaf vegetables, etc) and sponges, it won't be getting the right nutrition. The butterfly would appreciate a gel-type food smeared onto a piece of rock (let it "set" in the fridge before placing it in the tank).
