If I were raise the water level in the water intake and then make a shelf with eggcrate and put macroalgae on it would that make a good refugium or does macroalgae require more?

you could I guess, lighting it would be a challenge since most of your space is vertical. also there isn't a whole lot of space there for mass so the effect wouldn't be huge. probably be more work than its worth.

How much do you need to have an actual effect on the tank, what if I just had a handful down there in the water that grew out to fill the width?

any amount will have some effect, but you have to consider that mass = results so the more macro algae you can grow the more results in phosphate and nitrate reduction your going to see. if its a fun project for you, then go for it, it can't hurt. just make sure you set it up so it doesn't block your overflow or drain.
sort of off topic: are you running a sump or a canister?

I have a 20 gallon sump. Do you think its possible to grow macroalgae using a $10 15-watt flouescent bulb from the hardware store?
1guyDude-how much does a setup like that cost?

it sure is, I generally use clamp lamps $5 and spiral bulbs for cheato growing works great. if you have a sump that's the ideal place for a refugium.
if your running a sump putting anything inside you OFB like that would lead to a flooding risk. it's deinitely safer to do this in your sump.
