Killer Humu Humu?

Ive had a number of fish (well only like 2-3 but still) of fish die, and keep thinking I have something wrong with my water or something else, so i test it my self, comes out fine, take it to the LFS, they test it, turns out fine.
The only fish that I have no (in a 150gallon take) is a 4-5'' humu humu trigger. Ive mysterously lost a perc clown, a naso tang, and a dalmation puffer.
Is this possible or is there maybe something I am missing here?

It's absolutely possible, a 5" Huma Huma can do some damage.

that would really be one aggressive fish.. id say all you can do is get rid of himand try again and see if the fish survive then you know its him...

I had a 2.5 inch Humu Humu kill a 9 inch long Volitans Lion, so yes, those guys can do some serious damage...
