Lawn Mower Blenny dissappeared

I have a 75gal tank with the following livestock:
 
Yellow Tang
2 Black/White clowns
2 Green Chromis
lawnmower blenny
and two sandsifting star fish.
 
Recently, both starfish have died. I had seen both of them along the sand of the tank with all of their arms chopped off and then they both ultimately died. I have since removed them and noticed that my lawnmower blenny is missing. I have be looking behind the tank, in the overflow, around the rocks, ect. and I still cannot find him.
 
Is there any indication with my stock list what is going on here?
 
My PH is 8.4. and Temp is 78-79 steadily I will test the rest of my water parameters later but I have not changed anything in my tank for a few months.
 
The rest of my fish are eating and swimming regularly and seem normal.

Set up since January.
 
Have had the tang and blenny since march/april and the clowns and chromis for a year and a half and the starfish since june.
 
Is there either plenty of algae for him to eat or do you regularly put algae sheets in there? I wouldn't think any of your fish are to blame.

I usually put algae sheets out every 2 days. He looked fairly plump and healthy. I dont remember if I saw him actually eating the sheets. He could have been bullied and pushed out of the way when he tried to eat. But I cant say exactly.
 
But where the hell could he have gone? I have moved rocks and looked all around the tank with a flashlight.
 
Poor Guy, I named him Waldo cause he blended in with the rocks. Where's Waldo?

It's possible he is still in there and alive...I have seen my bi-color pseudo disappear for a week and reappear.

Hope so, I dont have many hiding places. Do they bury themselves? or dig holes under rocks?
 
As for my starfish, any ideas why their arms were eaten?
 
My nitrate is at 0 ppm Ph 8.4 according to my PH probe. SG is 1.022 All water parameters seem fine to me.
