I've had a few...
I have a refill line to my inwall basement tank. Started a top off of the tank, walked away for a second and got side tracked. Lost my isobar and made a nice mess. I've done this twice... (hectic life).
Turned off my skimmer for a few days, water siphoned out of tank through airhose intake - lost another isobar and a few inches of water.
Wanted to rid tank of a velvet damsel - had to take out every piece of LR to get the little SOB... another night of $%#$%#$%#.
Lets hear yours!

I had to catch a mean Bi-color parrotfish. I took out about 50lbs of LR. My "D-A", let the LR sit out for about an hour. After I put it back in, the die off caused a nice Ammo spike, and 2 days later the ammo killed my other fish........

Oh geeze, too many to list! Recently I dropped a brand new light in my refugium just a week after my housitter did the same thing. $80 worth of lights gone in two weeks.
Other than that your typical floods from the tank, let an expensive pump run dry. left a pre-filter off a HOB powerfilter and a fish got sucked up and killed

ouch... I've always wondered what it would be like to have my lights drop in while my hands are in there. Yes I know - my hands shouldn't be in there.

a couple days ago, i was drip aclcimating my new BTA, i went to work only to come home and find out the siphon tube slipped out of the bucket and dripped 4 gallons of SW all over the carpet....and it was taped down!

i've had boiled fish too. i had a batch of platy fry, came home from school, temp was off the thermometer

Yeah, my nightmare was spending over 100 bucks on my first 5 fish. Putting them in all at once (thinking my tank had cycled with just LR and LS) and then watching them drop off one by one. first it was the tomato clown, then the BHT got Ich. then the Kole, then 7 Damsels and finally my Yellow tang.
Never again, will I be so over zealous. I was like a kid in a candy store..."I'll take one of those, and one of those, Oh! look at the pretty colors...give me 3 of those....etc.

I've lost count of the # of times I've flooded the laundry room by leaving the RO unit going, having a faulty needle valve, or having the wastewater line slip out of the drain. It's a jury-rigged setup and boy howdy but you can tell. We've had up to an inch of water in there.

I've had a problem with leaks too, my HOT skimer leaked all night, around 7gal, drip by drip down the back of my dresser,(which my tank is on) . Had to move the whole tank to dry the spot, water is heavy

After drip accl my star fish i tried to pour it into the tank so it wouldnt leave the water.. It was stuck on the bottom, so trying to pour a 5 gallon bucket, I stop to reach in to get it off the bottom, while I do that, I drop the front of the bucket and proceed to pour a few gallons on the floor.
I drip Acclimated some stuff in a nice strofoam cooler....That had Many Leaks! Needless to say my carpet is acclimated..lol
Tried hand feeding my anenome only to get stung many many times!

I've done that - had fish in a bag in a styrofoam container, bag tilted over, water leaked out, through air hockey table onto carpet. Saved fish however...
At a previous place I worked I left water on in the sink and walked away, flooded whole room. After a few times of this it becomes almost funny.

Never try to clean your tank while talking on the cell phone. I was cleaning my tank while talking on the phone. My picasso trigger nipped my hand I jumped and the phone dropped right into the tank. Nextel blackberrys are expensive not to mention all my business contacts lost. From now on I use a ear piece while cleaning the tank.
