Doing sand sifting starfish destroy live sand. What I mean is do they eat all the organisms in the sand?

I have been seeing quite a few posts saying that they do lately.
With rocks and such, I can't imagine that they could get to 'everything,' but it appears as though they can do a pretty good job of it.
If I just spent $100 on critters, it would make me think twice before putting one of those in the tank.
