lose scales???

The only fish in my reef tank is a tomato clown and about a two weeks ago he had white patch on his top lip. It looks almost like it is a lose scale or something hang of. It went away for about five days and now its back. Could this be some kind of bacterial infection? and if so any suggestions?
The spot is about the size of a fine piece of sand. White in color and sticks outward from the fish's scales.

The spot is much too large to be ich. I meant larger grain of sand (sorry about the confusion). It sticks out from the front of his mouth right atop the upper jaw. Its hard to discribe, however it is not slimy looking. It would almost look like a white-head zit on a human. I'll check again tomorrow afternoon and try to give a better, more accurate description when the fish is more visible.
