Lost a head of my Hammer and Dont know why

So the last 2 days, one of my 2 heads of hammer coral havent been looking so hot. Today, you could see skeleton in the middle near the mouth. So I bit the bullet and used some shears to cut the dead head off. Now, the other head isnt looking so hot now. What do you think could have caused the death?
Heres the specs....
150 watt MH over a 24 gallon nano
37.5% turnover rate
Various stony corals, LPS, and softies, none in the range to be able to sting the hammer. Nearby buds are GSP, my clam, ricordea. All about 2" away at closest.
Ammo/Trite = 0 (0.05 from the decaying head, since removed)
Trate = 5 ppm
Calcium = 440
Alkalinity = 5.6 meq/L (slowly lowering, and it got that high due to math errors ><)
Maroon Clown
Coral Beauty Angel (never seen it pick at anything, but I think while Im not in the room, it may have picked at the coral.
What do you all think would cause the head to die randomly?

How long have you had it?
What kinds of softies do you have?
Does the clown ever host in it?
Are you sure that the CB never picks at it?
Any pics?

Ive had it for about a month.
By softies, I meant like yellow polyps, various zoanthids and palythoas.
No, clown hasnt hosted anything.
Im pretty sure, but he could when Im not around.

When I had my hammer if I changed the flow it would seriously mess with it. Also if I had it placed in 100% direct lighting. Mine like LOADS of flow all the time!

Yea, mines on the bottom on a rock with great flow and direct light exposure. The remaining head is opening fully and looking great. I think the other may have had a bacterial infection or something like that.

Thanks, Im keeping close watch. Ammonia didnt really spike either, and if there was a trace of it, nothing reacted in a foul way to it.

My first thought was that the Clown was probably hosting it but I see that was already addressed. Glad to see things seem to be calming down! Good luck

I wish my clown would have hosted it
. All my pieces are still frag sized, so he wouldnt host them quite yet. Maybe once they grow out.

for some reason, i almost lost a head of my hammer.
it was in really really bad shape. i was able to see the skeleton underneath the thin layer of flesh. all out of the blue.
i changed its position, and now its back to its normal size (about a softball)

no, thats the problem.
i was never able to find the reason. maybe it was acclimation to the new tank.
