My Balding & Dissappearing top fin Koran Angelfish

Hi there professionals; I have a question? I have a Koran Angelfish, first he is pretty large and hadn't changed like they usually do, Second over the past year he has began to what I call "Balding" on his face, Third it also seems that pieces of his top fin have been magically disappearing...I say this because I don't see pieces of it anywhere when I clean the tank or move around the rocks. I currently feed the tank every morning frozen small pieces of Krill (for the Blue Spotted Puffer) and Flakes (Ocean Nutrition - Prime Reef Flakes). He doesnt eat the Krill anymore ( he used to) he will only eat the Large Flakes, I have tried Mysis shrimp, Romain leaves too he will only eat the Flakes and the usual grazing of the rocks.
I have a few upclose pics of him in my album here - please take a look and tell me what are your thoughts?

It is a 55 Gal tank, with Fox Face, Cline Butterfly, Seabae Clown, Blue Spotted Puffer and a few damsels. He;s been like this for months....
