My frogspawn is falling apart

Can some one help me with this problem?

Temp Is 77.9 to 80 degress
1.023
Ammonia is 0
nitrite is 0.5
nitrate is 40
Can anyone help me?

The polyps are out and it looks pretty healthy to me what is the problem? I see a small piece on the sandbed is that what fell off?

salinity is low and you should have 0 nitrite what do you have for lights? how old is your tank

are all your lights on in the pic?is this your only coral and what is your ph and alk readings
 
Did you just move it or bump into it? Is there brown jelly coming out of the dead heads? If you have brown jelly try Vitiamin C. You can use human or fish which ever. I know a guy who uses human C when he has this problem.

Nitrates can be lowered with water changes and may want to think about running macro algea. So would you say it recceeded where you see that bare spot that looks like it was cut? How does it look today?

The last water change is when I noticed nitrates where to high. Yes it did receded from those broken looking spots.
In the last 2 or 3 month's I have lost some zoo's and Star polyps

This coral has been closed for a long time 6 month's. sorry about pic quality I kicked up some sediment right before pic was taken.

My brain coral has not opened all the way in 2 days.

Well somethings going on just finding out what looks like it's not going to be easy. If you can borrow someone's Ph and Alk + Ca test kits those parameters might help us. Otherwise just keep doing water changes as needed and then you could see if you could put some chaeto in there to help with the nitrates problem. Also cut back on your feeding if you think you may feed often.

55g with TEK light 44 48" T5s don't remember the lumins I do know that they are the most powerful.....
I moved the coral brain last night.

FINALLY GOT TO THE PET STORE AND BOUGHT A CALCIUM TEST. It reads about
580ppm. i do believe that might be to high for my soft corals. Is there anything out there to lower the levels?
