generally highly debated - anything under 30 gallons - imo
basically a tank where someone goes much smaller then the recommended size and does it successfully .... lots of good looking nano tanks - but they are more demanding!

Can a nano tank be considered a quarantine/hospital tank? Are you supposed to keep LR & LS in the tank? Can you have a fish or two in the tank if it's a quarantine tank? Is lighting an issue?
When bringing a fish from the LFS, how do you guys introduce them into the tanks? Is it in the quarantine tank for a couple of weeks, then to the main tank? I keep reading on these forums to quarantine your fish first, before placing them into the main tank. What are you looking for or trying to prevent. Thanks for your input.

IMO a 29 or less. highly debated though.
I'd recommend following the acclimation direction provided to us by SWF.com it's 3 buttons above message boards.
I guess you could consider a QT tank a "nano" but most people don't. You want the QT tank to be very empty. Most people offer PVC tubes as hiding places. Keep LR and LS out of it. This will make it easier to disinfect the tank. I don't think its the greratest idea to keep fish in your QT tanks as regular tank occupants. QTing a fish is meant to give any diseases time to rear their heads so that you can treat the fish away from the main display. Many people keep the fish in the QT until ~ 1 month w/ a clean bill of health, then they reacclimate the fish into the main display.

For a QT tank, is there a bacteria cycle? Since there's only water? I would assume the lighting is not as critical, not is a filtration system, right? Or am I fooling myself?

You are right - good HOB filter is all that is really needed. Youy do need a cycled tank to use as a QT tank. Best thing to do is do a large water change on your big tank - save a bunch of water and then mix it so it is about 50/50 fresh salt water and tank water mixed.
Check your levels and have it up and running for a couple of weeks @ 0 levels before adding a fish.
Patience is not only a virtue with SWF but it is a requirement!
