Need Help Asap

I have a Porcupine Puffer and its just laying there for the last 4 days not eating and his or her eyes are getting cloudy.........

what are your water parameters? anything off? do you have a qt tank?

Is this a new fish. Any signs of ich? Are both eyes cloudy, or just one?
