New Tank Set Up PLEASE HELP!

I have recently purchased and set up a new tank. It is a 29 Gallon bow front. I have a hang-on filter as well as live sand. I also just added a little more then 2 #'s live rock. I purchased 4 fish : 1 false percula, 1 4-stripe damsel, 1 tomato clown, and one i'm not sure what the name is but he is purple up front and goes to almost see thru at tail
I noticed there is a problem with the 4 stripe
He has what appears to be either fin rot or ich
I just bought a cleaner shrimp to hopefully take care of the bottom
I added Melafix to the tank. I really do not like to use chemicals but I had to use the cycle as well as the melafix to try and prevent the spreading of whatever is wrong with stripe. He also grew a white spot on him over the course of one nite.
any help would be greatly appreciated.

tank was set up saturday nite
these are starter fish for the tank to cycle
lfs told me to purchase these to start as well as everything i read on the web told me to start with dasels to cycle ... they are very hardy
not sure how to reply to the posts so hopefully this gets to where it is supposed to go

ph is between 8.2 and 8.4
nitrite is 0
ammonia is between .25-.50
nitrate is between 0-5.0

the 4 stripe had lost some color i just noticed when i retrieved the color chart for the water

i also have biocem stars in the filter and have removed the charcoal while adding the melafix

on the one side there is a white spot
on the two underside fins one looks multicolored and ragged
the other i haven't seen at all today like it is being tucked away
i have aim instant messenger or icq or yahoo messenger to transmit the picture if that might help

he has been eating as well as hiding under the dead coral
he is not labored breathing
the lfs tested the water and said it was fine yesterday
I am also confused as to the different readings on the water test
what i have read so far is ammonia breaks down to nitrite and then to nitrate ... if that is correct then why is there no nitrite showing?

i just don't want to not be doing something that i should really be doing first thing in the morning new to salt and nervous

salt water master test kit
oops used tap water ... might be the problem
