I have lost two fish a tang and clown fish, and the clown fish was all the way in the power head. The tang was still on the outside but stuck to the power head . Is there an attachment to put on the intake of the power head to prevent this ?
ditto. I have had fish sucked into PHs but in each case the fish was weakened already.
they do make sponge intake filters for PHs. You might want to check those out.
But you also need to see why the fish do not have enough strength to stay outta the PH to begin with.
If the power head would only take the weak then I have to find out whether the Blue Damsel or the Lawn Mower Blenny got aggressive at night and distressed the other three that I lost , a yellow tang , sailfin tang , and a clownfish. Ive checked the water and its good all the way around. Anybody got any guesses ?
did you just find them stuck to the powerhead? If so i am sure that a healthy fish would be able to swim against the current to get away. IMO the fish was either already dead...or dying.

I'd definitely suggest some sort of sponge cover on the PH intakes...its not only stressed or sick fish that can get caught...healthy ones can suffer from exposed PH intakes also.
I have had a perfectly healthy fish get caught in the intake of a power filter, so I do believe one could get caught in a powerhead. As was said before, figure out a way to cover the intake of the powerheads. A bioball or some sort of mesh covering with a rubberband, you will just have to remember to change it or clean it out when it gets dirty and clogged up.
Ya know, about 3 yrs ago, right after we got our lmb, it got stuck to a powerhead. After we spotted it, we turned off the unit and it got away. Now, today he is a bit larger. So I sincerely doubt he was dying.
