Really good book?

What's a really good, modern book that explains how to set up a successful reef tank, to include all equipment, and contains a discussion on corals and reef fish?

I agree. Natural Reef Aquariums by Tullock is good. Great pictures and he has a step-by-step process of setting up a tank.
Good old (that aren't really that old) standbys are The Reef Aquarium Volume One and Two by Charles Delbeek and Julian Sprung. These volumes are expensive ($65+ each), but are good staple books to have on your shelf.
For the money ($ 25-30)Tullocks book is where I'd start.

The Reef aquarium Vol. 1 by Delbeek and Sprung is good, also the Modern coral reef aquarium Vol. 1 by Nilson and Fossa is good. Both of these books contain TONS of information, and will tell you more than you could ever want to know. The Reef aquarium Vol. 1 has all the information about setting up the tank, and gives info about clams, and hard corals. Vol. 2 covers soft corals. A third volume is planned. The Tullock book is good also. It is straight-foreward and easy to understand.

I think for someone new to the hobby that Natural Reef Aquarium by Tullock is by far the easiest to read and understand. It's full of useful step by step information and is well illustrated. All the other books suggested here are excellent also. I have all the books that have been mentioned here so far (and then some) but if someone new asked me for a book, Natural Reef Aquarium would be the one I handed them.
