Red Fungus on Yellow Tang??

I have a yellow tang, Around his face and eyes the skin seems to be coming off, I have a rid volt titanium ground.
Also there is a red fungus on the back of the tang. It looks like red fungus. I have a cleaner shrimp, but I have never seen the shrimp clean the tang. I do have Green Damsels that cleaned.
What is this fungus and skin problem.
What do I need to do to fix it?
All the other fish abd tank critters look fine.

It sound to me like head and lateral line erosion with a bit of a bacterial infection.
Can you give some info about your tank. Size, type, water readings, age? What type filters do you use? How long have you had the tang? When is the last time you did a water change? What are you maintenace practices?
