Red Spot On My Cali

Hey guys, bail me out on this one. Two days ago, I noticed a peculiar red spot on the outer most edge of my ray's disc. Today, it seems to be a bit larger. Upon turning him over, I found that there was the exact same mark on the other side as if something went through it. All I can think is bacterial infection. Please help.

Do you have smooth, fine substrate, if not you'll have repeated problems like this?
Does he have ample free swimming room free of rocks, etc?

Yes, he has a 2 1/2" bottom of live sand, and about 4' of open sand broken up onboth ends by caves, and then another 8" of open sand. The structures are only 10-12" tall.
