sounds like your tank was not fully cycled and rock not fully cured how long was this tank set up for

Definatly the Rock...smells like low tide I bet
Rock was not cured.....What is Amonia level.
I guesss the trigger never had a chanche to eat that urchin....Triggers love sea Urchin BTW!
Dont give up do a water change....wait a week...do another until it gets better.

you need to be checking your nitrate, nitrite, ammonia and phosphate levels,, when everything is down to zero then you are good to put a fish in,, check levels again over the following 2 weeks then you may be able to add something else (sometimes you may have to wait a month or more). Also,,,what kind of filtration do you have?

what other equipment do you have on this tank? depending on the size of the trigger, the tank you had it in could be way too small for it in the long run. if it was a larger fish, was it doing anything odd before it died? it does sound like your tank wasnt cycled, but there are other things that you may want to do differently.
