Should I worry about my seabae?

It wandered into a goby tunnel a few days ago and settled tenacles underground, foot end at the mouth of the tunnel. I've been keeping an eye on it and noticed this morning it's foot looked like it had little skin tags on it. Knowing a dying anemone can bomb a tank, I gently removed him. His mouth was gaping and looked like cauliflower in the middle (sorry for the blue). I figured the worst and got stuff ready to pull him out of the tank, in which time he closed his mouth and started puffing his tenacles back up. He is also still sticky. I left him for the day and SO says he's looking fine now.

Is this worrisome or should I just continue to keep an eye on him? I'm at work now, but params were all within ranges 2 days ago and have been steady. The tank is 4 months old. Yes I'm a noob and I know (now) that ideally we shouldn't have gotten a nem yet, especially a seabae. He's been eating well, draining and filling nightly, up until he went face first in a hole. We also have a green bubble tip that seems to be thriving on the other end of the tank.

Edit: I pulled it. His mouth was still gaping when I got home and his foot was starting to look like it had been through a grater. ☹
