I have a leather toadstool that doesnt have a trunk,stalk, base call it what you want I dont know. About 3 months ago it was 3"to4" tall no longer. It has a full head and full extension. Any clue what is causing this and is it a problem or not.

Dont get you question. It HAD a 3-4" stalk? And USE to stand tall. And now doesnt?
Check your water params if thats the case. Something must be off. Unless you changed up the lighting.
Check for to much flow possibly hitting it?

Water params had done nothing but get better. I did add 250w of mh and it does sit about 5" below the powerhead. Does anyone know if this is detrimental to its health.

Well when switching lights it is highly recommended you acclimate the entire tank to the lighting.
There are different methods to doing so. But for example if you went form pc or t5 lighting to a 250 mh... Your definately going to see some changes in your corals without acclimating.
