Sick Heniochus

About 2 days ago I noticed some grayish blotches on my henni. they are on both sides near the front top of part of him, near where the long fin starts. Now he has 2 tears in his long fin also. he is eating ok but he keeps twitching alot when swimming also??? I alternate feeding him brine, seaweed selects, spirulina, bloodworms, and flakes. My tank is 75 gallons FO and only other fish in it are 2 false percs.(so i don't think it's an injury) they seem fine. Last nite i did a 30% water change and my parms this morning were:
Temp 80
Salinity 1.020
ammonia 0
nitrite 0
PH 8
nitrate 10ppm
Anyone have any suggestions??

the false percs like 7 months the heni about 2 1/2 weeks( i do not have a QT) The only fis h that seem to live are the false percs. Everything else always seems to die and my water parms always test perfect?? Just FYI the heni is still eating fine, ate brine earlier and when i tried adding a new chunk of seaweed to his clip he starting grabbing it before i even had it stuck to the glass!!

I did all that above, in the first post except for equipment.
Marineland bio-wheel pro 60 wet/dry biological, penguin 1140 powerhead, 250 watt marineland visi-therm heater, coral life 50/50 lighting, Live DSB, no live rock and not a reef tank. The ONLY inhabitants are 2 false percs and the 1 henni. The tank has been cycled for about 11months, the percs have been in there about 7 months without and health problems ever. Unfortunately everything else I add does . As far as water parms and symptoms they are still the same as first post above. TY
