Sick Yello Tang

Beth - can you help??
Yellow Tang sick - please help!!
I have a yellow tang that I purchased two weeks ago. First he got black spots all over him. I treated the tank (can't remember name of liquid) its used for treatment of ick. Replaces slime coating. This did the trick and he was much better. Now one side of him has a dark spot and one eye seems to be covered with a film.
I have lost almost eveything that I had in the tank. I am extremely depressed about the Salt tank. I have taken water to too different LFS and both have said that the levels are perfect, nobody seems to know why everything keeps dying. I have a 125 Gallon that I wanted to set up as salt water but am so disgusted right now, that I am tempted to set it up as Fresh water.
Please can anyone tell me what to do to save my yellow tang.

My educated guess would be the meds that you put in the tank caused the problem. What is that medication?
Have you read any books before getting started in the hobby? What type of tank do you currently have set up?

Don't remeber the name of the meds. It has a yellow tang on the front. I am at work so can't check right now.
Yes, I did read every book I could get my hands on, and followed all of the advise given by my lfs. I have now found out that alof of the advise I was given was bad, from the lfs and ended up with fish eating other fish.
I have a 55 Gal. 20 lbs lr and 3 -4 inch ls

Star, what are your water readings, and what other fish do you have in the tank? Do you have LR?
The black spots that the fish had before....was it like the yellow tang picture in the Diseased Fish Thread [top of this forum] that has black ich? Please take a look and confirm.
Also, please give details about what you are currently seeing.
