Spanish Hog is Sick, Need Help!

I have a small Spanish Hog that has been acting weird for the last few days. Its colors are getting dull and it has been laying around a lot more often. It just sits at the bottom of the tank and rarely moves. Its breating really fast and hard. Its not eating either. I've tried flakes, frozen bryne shrimp, algae sheets and nothing. My water parameters are ok, nitrate is a bit high (40 ppm) but its been that way since I've had the Hog. I'm afraid if I dont do anything I might lose this little guy. Any ideas? below is some info on my tank:
55 Gallon w/ wet/dry
50 lbs of LR
40 lbs of LS
1 Maroon Clown
1 four striped damsel
1 spiny blue lobster
1 emerald crab
several hermits

How long have you had this fish? Notice any redness to skin, torn fins, spots, red gills?

I've had him for just under a month now. His fins are intact and he has no spots but his colors seem a bit faded. He seems to be getting worse. All he does is lay on the sand breathing hard.

The poor guy didnt make it. The water parameters were ok, all the other fish and creatures were perfectly fine. Any ideas what it could of been.
