I just got a 220 with a built in over flow into a 40 gallon sump. What come after the sump? Do I put anything in the sump? (fish only, some live rock)

You can just leave it like it is. With the sump there you have a place to put your protein skimmer.

What about a carbon filter or a bio system? I was thinking of adding a canister filter. Is this a bad idea?

Depends on the amount of Live rock. If you do not have enough for sufficent Bio-Filtration you will ned something for the bacteria to grow on in the sump. If you have lots of Rock in the tank and do not plan on a heavy Bio-Load you will be fine with an empty sump.

The DSB is a much better bio filter the LR. Once it's established you'll be fine. I'd skip the bioballs myself.
Also you can put your heaters in your sump, it's a nice plave to hide them.
Throwing a light in top and putting some macro algae in the sump will help to control hair algae.
