Tank troubles

My parent’s tank has been setup for around 5 months and lately there have been a lot of problems. First off it is a 100 gallon plenum filtered tank with power heads that make 10x water movement. There is 75 lbs lr and cc and sand substrate. The problems started about 2.5 to 3 months into it. After the cycle was complete I moved a 4" maroon clown and 5" double saddle butterfly into the tank and they were fine for 3 weeks. When then added a bicolor angel. The next day the butterfly died of what seemed like stress or being picked on by the angel so we took the angel back. A couple days later the maroon clown got what looked like marine velvet, or ich, her skin became flakey and white and started to peel off. We treated her with formalin 3 and she seemed to get better but died within that week. We waited 3 weeks from the advice from our lfs thinking that if it was a parasite without a host it would die. From there we got a panther grouper which is still alive and well 4-5 weeks later. a week and half after that we got a ob batfish that developed what looked like ich and pop eye. it got better for a week then died within the next week. a couple weeks later we got a coral beauty angel that within two days got ich and died. we had been doing water changes of 20% at least every 3 weeks, our water always tests out perfect, 0 nitrites and 0 nitrates, 8.3 ph. it just seems like whatever we out into the tank develops some sort of ich and dies. Does anyone know the reason for this a ways to treat it?

Do I need to worry about taking out the snails and crabs? Can the pathogens live off of them or only off of fish? What is the minimum tank you recommended for a qt?

it's been a couple of days since i took out the last fish so that that tank can sit idle for 5 weeks, but when 5 weeks are done should i do a water change before a fish is added? and i will qt the fish for at least 3 weeks, this time.
