How high off the tanks sureface do the lights need to hang? Also, do I need to leave the tank open under the canopy or is there some kind of top that I need?

It all kind of depends the lighting you have and your personal preference. Lids are great on a tank but they limit airflow over the surface which is a major place where evaporation/cooling takes place, so you'll have to see if your tank stays at temp with one. For light distance from the surface six inches is usually enough (sometimes overkill depending on size of tank) for any form of flourescent bulbs but you might want closer to 8-10inches for mh. This is partly because single ended bulbs have no protection from water splashes etc. I'm sure others will have their own personal preference when it comes to this.

yes, it would depend on what lighting you have
although, dont get a top, not onlly would it heat up your tank like all heck, but it will limit gas exchange and cause you problems. If you relay want a top to prevent fish from jumping, get some eggcrate.
