thoughts from the experts?

Hey everyone! I"m just looking for some afterthoughts on what could have happened here.
My 3 fish were happily swimming along after 2 weeks in the QT (don't worry, I hadn't removed them) with just the slightest sprinkling of ich left. The SG is down to 1.009, water changed every couple days to keep it clean. All were eating well, playing well and just being fish. The last couple days however things went downhill fast. I had decided to treat with furacyn because of some scales peeling off and fins splitting (treatment instructions actually received from this forum, not the package) and do another water change. This morning I found two of the fish dead with major major slime coming off their bodies and as well out of their mouths. The third died later on today with the same symptoms. When I went to retrieve the body with a net it was almost like the skin was just sloughing off completely.
In my virgin saltwater days I had this happen once before but I didn't know about hypo and 4 week quarantines (which I learnt about FAST
) and had a whole quarantine tank die off with the exact same symptoms. I attributed those deaths to being treated with Nox-Ich at that time and a small QT which I only used for less than a week before returning the fish.
Any thoughts on what happened or what contributed to the cause of death? I'd hate to mistakenly follow the same process that caused those deaths if I ever have to QT in the future. As it stands, I'm leaving my main tank alone and letting it thrive with the few inhabitants there are.

I used the dosage you recommended in another post to someone with a sick puffer (I believe). It was 30 mg/gal that was suggested. My qt is only a 10 gal so I added just the 3 caps (1 mg each). I stick to RO water for any water changes/top ups, etc. I admit I was more concerned with the ammonia and nitrates rather than the pH so that is a possibility that I may have let it drop too low.
Well the tank has been completely cleaned out anyways and my poor baby yellow tang who was stuck in a smaller qt has been moved into it for the next few weeks. At least she's a tiny thing so it didn't hurt as much to move to a small qt. You try explaining to the significant other who thinks fish should be eaten <g> why you need a 5th tank in the house (FW here too)
