trigger not doing well

my trigger is floating around upside down and not swimming
he has done this before
can anyone help?

how long have you had him and what are the parameters?

he has died since i posted the thread....
i had him for over 3 years and all the water test are o.k
he was with a rock cod two lion fish and a honey comb eel.
not a good day for me lost a snow flake eel and a parrot fish that where in my other tank

Very sorry to hear of your losses. Am I understanding this correctly...you lost the trigger, eel, and parrotfish all in the same day?
