VHO Ballast Wiring

I bought a Fulham Workhorse 7 Ballast that I am planning to use to power 2 110W VHO bulbs using two dual icecap water-proof end caps. There was no wiring diagram with the ballast so I need some help wiring it correctly.
There are two sets of two red wires and also a single yellow wire; five wires in all. The other end of the bellast there is one white and one black which will be plugged into an outlet. I think from looking at the fulham website that each set of 2 red wires get spliced together and go to both pins of one end of each bulb. Then the yellow wire is connected to the opposite end of both bulbs. Does this sound correct?
Also, how does the entire thing get grounded? Does the ground wire from the power line get connnected to the ballast's case?
One last question....I'd like to extend the red and yellow wires so that I can mount the ballast on my stand instead of my canopy. Is there a max length of wire from the ballast to the bulbs?
